Mountain Comprehensive Health Corporation (MCHC) partnered with atom Alliance, the Medicare Quality Improvement Organization for Alabama, Indiana, Kentucky, Mississippi, and Tennessee, to improve access to diabetes educators in its southeast Kentucky service area that has very high rates of uncontrolled diabetes. MCHC worked with atom Alliance to train six of their employees to be Peer Facilitators, who then provide patient and community education. To date, MCHC has had nearly 200 people attend at least one of the Diabetes Self-Management Education and Support courses. One of the Peer Facilitators has also become a Licensed Diabetes Educator. MCHC has experienced such great success with the program that they have referred other non-profit, health-related groups to atom Alliance to spread the great work.
