The Kentucky Health Center Network is pleased to present our 2024 Annual Report. The report summarizes our efforts to support the impactful work of the health centers we serve. Throughout the year, we supported our member Community Health Centers to:
- Enhance patient privacy and security,
- Increase patient engagement and digital health literacy,
- Support data utilization, disaggregated patient-level data reporting, interventions for populations of special focus, and interoperable data exchange and integration, and
- Leverage and improve the use of digital health tools.
This report highlights the key initiatives, metrics, and milestones that shaped our year. It also honors the participation of the dedicated staff of member health centers who demonstrate daily their ongoing commitment to clinical and operational quality to best serve their patients and communities.
