KHCN is pleased to announce that we had a very successful operational site visit in December during which all grant conditions were deemed as Met. The site visit is conducted to review program compliance. KHCN’s peer learning model was identified as a best practice. Other comments from the report include, “KHCN has strong executive leadership, an engaged board of directors, and a talented staff team. Throughout the site visit, health center staff provided examples of the excellent support they receive from KHCN. Its board and staff leadership bring a thoughtful perspective and have worked together to ensure the HCCN (Health Center Controlled Network) is a strong organization that responds to the needs of its PHCs. “

Operational site visits are conducted by HRSA’s Bureau of Primary Health Care at least once during each three-year grant period to ensure program compliance through an in-depth review of the structure and oversight of the organization, including active engagement of Participating Health Centers and progress made on all grant objectives. Grant objectives for 2022 – 2025 are patient engagement, patient privacy and cybersecurity, social risk factor intervention, disaggregated patient-level data, interoperable data exchange and integration, data utilization, leveraging digital health tools, provider and staff well-being, health equity, and improving digital health tools.
